my yorkshire terrier has a lump underneath the skin of her nipple what could it be

Take her to the vets as soon as possible.  It is probably nothing serious but could be mastitis or a tumour. 

I don't wish to worry you though, as my dog had the same and it was a gnat bite! 

However, please do not delay your visit.
