1. Remove road wheel
2. Remove brake caliper. You may have to force the brake pads back a tiny bit to get the caliper off. Don't push them right back!
3. Remove brake disc. Often it is retained by screws. It may need some persuasion with a hammer to remove the disc.
4. Ensure hub is clean, put some copper grease on if available and fit new disc. If new disc has paint over it then give it a good rub with some emery paper (industrial sandpaper). Ensure all clean and fit disc.
5. If old disc was warped then it might be worth fitting new brake pads as well.
If when refitting the brake caliper it won't go over the disc (because the gap between the pads isn't wide enough) then you have a couple of options. Check the brake fluid level under the bonnet. If it is very full then you can either bleed some fluid off the caliper as you move the pads/piston back - you'll have to bleed the brakes this way. There is another, easier, but bodgier way of doing it but I won't say at the moment! :)
