Incredible Hulk

In the last ever Incredible Hulk ((approx 1989), did David Banner die as himself or as the Hulk ?

As I remember it, he fell from from a plane (or helicoptor) after a struggle as the Hulk and gave the runway a nasty new crator, and then returned to Banner after death.

I am pretty sure he died as the hulk as well. I remember reading it when he died in 1993 approx.
