If it's a man you're thinking of - for 30 you can buy a square foot piece of land in ~Scotland and the person becomes the Laird of that area and they get a certificate to that effect and a map showing where the land is.
People also have stars named after them but I think that's a sham scheme and it's not officially recognized.

Another idea, depending on how much you want to spend - one of those red letter day adventures. Driving a car round Brands Hatch, driving a tank, James Bond experience - loads to choose from.
Perhaps a flying lesson or a helicopter lesson - we did the last one for my son and he absolutely loved it. Although if this person has everything, he probably has a pilot's licence.

The finest gift you can ever give anyone is your time. Buy tickets for a show in town, or a football match (especially Everton great team!) :-) or one of my favourites if I want to offer someone a gratuity for a favour, is pay for them to have a meal at an all you can eat restaurant with a couple of bottles of wine thrown in (business only on that one though)
What are their interests? Walking, birdwatching, gardening, photography, sport? There is a magazine which caters for virtually every taste under the sun these days and possibly a year's subscription to a magazine which reflects one of these hobbies would be gratefully received. Researching magazine subscriptions on the internet might give you some ideas.
You could get a personalised gift? It shows that you've thought about what you've got and it's not something that one might necessarily buy for oneself... Have a look at www.WineGifts4U.co.uk, they've got some really good personalised labelled wine and champagne gift ideas. You can add your own photo, image and message onto a label, decide which bottle you want it to go on and then order it. They're quite affordable from 10 per bottle (plus p&p). They do personalised mugs, design your own t shirts, personalised ales, design your own teddy bears and engraved gifts. I think you'd be able to find something suitable for someone who has everything or can buy whatever they want!
