Can I claim the Health in maternity grant, even though my baby is 5 weeks old?

I know the Sure Start maternity grant is still "Claimable" after the birth of the baby, but is the Health one? I know I can still claim the vouchers, but not sure about the £190? My Midwife was "flakey" with regards to getting me the form!!!!!

....the answer is "NO" just phoned the helpline!
