Since the fashion for tight hosiery meant that a man's 'bits' could be seen, the designers of the day decided to make it a feature, and men wore a pouch made to make their genitals look bigger, and therefore the man more masculine.
So, you see a costume, fitting for the period in which the ballet is set, and not Mr N's actual 'bits' which are unlikely to have been quite as impressive.
