Name Change

Hi all, this is a bit of a long one really, and I really hope someone can help...
When my fiance was younger (About 3/4 years old), his mum and dad got divorced, and his mum changed his name back to her original one. His birth certificate still says his farthers name, even though everything like his passport/driving lisence etc say his mothers maiden name.
We have had a daughter and we have given her my husband-to-be's name, with the result being I will join them next year :) however, my fiance (Being the pain in the elbow that he is) has decided that he would like to consider changing his name back to his farthers.
This is where my problem begins.... How much is it going to cost for him to change his name and how do we go about it? I know we would have to change his passport and driving license, but is this going to cost much? and who else would we do it with? Also, we would change our daughters name, so is there anything that we have to do extra for this and is there any way of getting her birth certificate changed. (I am aware that after we marry, I can change my name on her birth certificate)
I really think he wants to do this (and I know it would mean SO much to his father), but I think he is a little bit put off by not knowing where to start and how much it is going to cost.

Was his name change at age 3/4 done by deed poll?
It is cheap to change your name by deed poll, even cheaper if on certian types of benefits, but to do the passport name change it costs the same as if renewing it

If it was never done legally he will have no need to change it back by deed poll - saves a little bit of cash
However, I'm thinking it must have been for him to have his passport in his mothers name

I would assume that if he got a passport he must have some sort of official certificate saying his name had changed from that on his birth certificate. So he has effectively at som point changed it legally.
If so then, I would have thought, he is quite at liberty to change it back again albeit at a cost
