To remove your points from your driving licence you will have to fill D1 ‘application which is available from the Driver and Vehicle Licensing Agency (DVLA) form ordering service or any Post Office® branch also
Also you will need to enclose your photocard driving licence, paper counterpart and the appropriate fee of £20.00
I f your name has changed you'll also need to provide original documentation confirming your new name
And send this all your documents to DVLA, Swansea, SA99 1BU
And to renew your photo card you will need:
1) To fill the photo renewal form D798
2) Both the photocard and counterpart of your licence if you have them
3) fee of £20.00
Hope this will help.
