For more on marking an answer as the "Best Answer", please visit our FAQ.The phrase comes from American commandos whose uniforms have a (ahem) 'exit flap' in the rear of the trousers to facilitate bodily functions when in hostile territory, i.e. anywhere there is no toilet to use. It makes sense therefore to eschew undergarments, which hinder the use of said apparatus. The phrase cropped up in an episode of the TV Comedy 'Friends' a few years ago, and has passed into popular usage from there.
