my sisters son was breastfed on demand and was not given dummies and he started sucking his thumb.

IMO,
they shouldn't have one at all. We had 4 children and none of them used one.

My midwife recommened a dummy for my LO as he sucked alot for comfort. Made me very sore and kept being sick from over feeding!
Every baby is different. I know many people who didn't give dummies and thumbs/fingers were sucked but also some who didn't suck anything at all!
IMO - stuff what other people THINK is right and wrong and make your own decisions.
