If the receiver puts the phone down they could be going to pick it up again in another room. The call will stay up until you clear it.
As far as I'm aware if one party disconnects but the caller doesn't the system checks for a voice signal on the line, and if there isn't one it disconnects after a short period of time............you should then get the 'hooter' down the line to tell you to replace the receiver.
I sometimes get a voice down the line saying 'the other person has cleared'
I may be wrong but I think that noise occurs when a receiver have been knocked off the phone and yet no number is being dialled. There may be a timeout on a call you forgot to end, but I am not aware of it.
Well that's what I was told when I worked at BT OG...........but I'm very gullible.
Hiya Craft. In our Exchange we used to call that the howler....a wailing sound that got louder and louder to draw someone's attention to the phone.

Don't know if they still use it now....I'm going back to the 70's.
