in previous centuries - it was said, without evidence because things were like that then, that the soul left the body during a sneeze and the Devil could take up residence in your vacated body !
So people said bless you! just at the moment because the Devil could not stay in a hallowed place !!
This was clearly a Christian duty and not a favour and one did not say thank you as an answer to bless you (but when you were young you were allowed a wish)
That last paragraph reminds me. One day at work, I happened to sneeze and a female colleague of mine kindly said "bless you". Being a polite chap, I replied "thank you" whereupon this normally sane, rational young lady gasped in horror and clapped her hands together three times in rapid succession.
"Erm, what was that all about?" I asked.
"You mustn't say thank you when someone says bless you," she replied, wide-eyed with disgust, "every time you do that, a fairy dies."
"I see," I said, edging nervously towards the door, "but what's with the clapping?"
She looked at me pityingly as if I was stupid. "Well, that's the only way to bring them back, of course", she snorted.
How silly of me.
