Apollo 13 - major malfunction - oxygen leak I think
Apollo 13 had an explosion in an oxygen tank leading to a loss of power. I seem to remember they fixed it with a kind of blue peter like fix!
I always think of it as Tom Hanks in Apollo 13 but a bit of googling suggests it's misquoted. Bit like 'play it gain Sam'!
Actually said by Jack Swigert, but they gave the line to Jim Lovell (Hanks) in the film.

"Houston, we've had a problem"
It was something like "Houston, there's a problem" but Ron Howard explains in the documentary that "Houston, we have a problem" sounded better. It was caused by a faulty coil in the fuel tanks which caused an explosion when the tanks were "stirred", a totally routine procedure.
