How do you tell a person that their standard of diving is poor when that person is your spouse.

I can't drive anymore as I'm in a wheelchair and am reliant on her to take me everywhere. I can't get an automatic fitted with hand controls as she won't drive an automatic.

why dont you mention to her that she seems nervous on the roads and buy her a course of 'refresher' lessons?

because however you tell her, just telling her wont change anything except to upset her and make her even more nervous of making a mistake because her confidence will be shot and she will feel she is being watched

so just telling her is pointless - do something to fix the problem ...otherwise say nothing
