As best as I recall, the whole thing came to light through the pro-active monitoring and testing policies that the Food Standards Agency - Ireland routinely carry out - Several frozen processed beef products tested positive for Horse DNA adulteration, and the whole thing snowballed from there.
FAS Ireland still carry out proper testing protocols, unlike the FSA in the UK.
