Because that is more or less what the dish looks like...namely, a rounded thing tucked into a hollow. Fair enough, it's a sausage in a pudding/crust rather than an amphibian in a hole, but the appearance is what counts.
I was round at my cousins once whilst he had a french girl staying with him, his wife made toad in the hole, and he translated it literally to the french girl who looked horrified and refused to eat it until he hastily explained that that was what it was called, not what it was!
I have just put a toad in my hole and it looks nothing like that.
