It may have happened in the gap between last series ending and this one beginning. We are meant to take it as fact, even though we didn't 'see' it.

Glad I'm not the only one trying to recollect something I never saw....

I was wondering the same, though I'd missed an episode somehow, even though I was sure I hadn't.
