has anyone received a form for housing benefit that offers Discretionary
housing payments, and if so can tell me what the criteria is, i don't want to link to the council website, just any knowledge or advice at this stage,

i'm afraid it's something you are going to look into yourself. AFAIK Each council can apply whatever criteria they like, so there is no answer to your question from people who don't know your circumstances. If you don't want to give any information to the council, fair enough, but it just means you absolutely won't get it rather than "may" not get it
