What is the law/rules on claiming ownership of land by "adverse possession" ?

Fence around it, maintain it (cut the grass/weeds twice a year), take the occasional picture to show your activity, wait 12 years, claim it by writing to the Land Registry. Post here again in 11.9 years for info on how to do this final bit.
