What a good question gordie and i am only going to guess at the answer based on my experiences.
If the heart and blood vessels (coronary) are healthy, then immersion in cold water will not harm the person involved. Remember that in WW2 torpedoed merchant navy seamen could survive for 30 mins in the ice cold water s of the North Atlantic.
However, the effect of throwing ice cold water over a person with impaired or "second hand " coronary artery circulation could well be fatal..

I ran from Glasgow Queen street to Central station one night 15 years ago when I was 27 after 5 bottles of lager and never having done any exercise in my life. It was 1/2 mile and I made it in 5 minutes and I made my train but my heart went into overdrive and I was taken to hairmyres hospital in East Kilbride with atrial fibrillation the next morning after being in it for over 9 hours with a resting heart rate of 165. I was put on a drip and it was 24 hours before I came out of AF. From then on I always maintain that exercise is bad for you!!
