Please would someone explain what the terms, subjectively and objectively mean when talking about mood? Eg, subjectively low but objectively elated.

S Subjective - what the patient told you.
O Objective - what you saw, heard felt, smelt or measured for yourself.
A Assessment - What conclusions S and O lead you to.
P Plan - what you are going to do about it including when you are going to re assess.
