Avoid surgery like the plague. Especially something so severe. And I'd mourn the body that I lost and was now incinerated, or rotting. Mentally it'd be awful. Now if they could convince my existing body to rejuvenate/change and be healthy and fit, that could well be a different matter.

If it were safe, and if the new body was genuinely preferable for some reason, then yes, but I'd probably need some serious incentive eg irreparable damage.
