Doorstep deliveries are at about 7am, and most newspapers also refresh their content at about the same time.
Those will be the two scheduled refresh times, but obviously if their is a breaking story, the websites can react to it almost instantly.

no, they add stories all the time, just as you might do if you ran a blog. They no longer have to wait until they've got all the stories of the day together, to be published all at once on paper. Any story can be published at any time, and that means as soon as possible.
That's not to say everyone does it; small ones with limited staff will just do it when they can. But big ones will always try to be first to publish a story.
(As an example of how this can go wrong, remember the Mail publishing the Foxy Knoxy verdict and getting it wrong. They prepared a Guilty story and a Not Guilty story and published as soon as the verdict came through - but they didn't understand the verdict and published the wrong one.)
