Is it acceptable for a care home carer. On there day off to take a resident out to the shops and go to the pub for lunch paid for by the resident. Does it need to be recorded to the care home manager as a gift .

To be fair, it might be a genuine kindness on the part of the carer. People, whatever their status and cognitive abilities do enjoy doing "normal stuff" and might enjoy "treating" a friend as a repayment for helping them to do something they otherwise couldn't do. Yes, a tactful word to the manager is probably your best way forward.
