DI boxes don't have a standard wiring, as they're for impedance matching, they're not just an adaptor.
What college course are you doing / where? I'm doing the same thing right now, we've been building cables etc for an assignment.
You also want to be careful re 5-pin DIN when not used as MIDI plugs - there's no standard wiring for them as audio connectors, so you have to check on the equipment itself.
6.25 and 3.5mm (Standard sized and 'mini-jacks') and bantam jacks are the same wiring, only one's smaller.
