can you train a cat to toilet outside!

Have a cat who i took in nearly 8 years ago, she was just a kitten and very traumatised,she uses a litter tray but sometimes in the summer she will stay out in the night for some hours, come back...

I'm a carer and a friend to an old chap.. his mobility is now very bad and I think he needs 24 hour cover ...should I ask him if he would go in to a care home .. if I can find one he would be happy in...
