I don't believe that a written note on your medical records at a hospital is going to be available to you in any medical records at the GP.
If it's a note in your hospital file then you need to send your request in writing to medical records I think, PALS would be able to advise, and there will be a fee. I don't think you would be able to get just a part of the notes as that relies on someone knowing exactly what to look for so you'd probably have to pay for the whole set.
I have known cases where notes have been read by the patient but someone has to sit and stay with them. I've never arranged that though so not sure what cirucmstances that's done in.
PALS will be your best bet in terms of advice, call them and go from there.
