Permission To Share Information With My Wife & Daughter

I would like to give permission for the GPs at my local health centre to discuss any aspect of my health with my wife & daughter. My wife also wants to give permission for her information to be shared with me & our daughter.

Does anyone know if there is a standard form that we can complete for this purpose? Thanks.

I think each surgery has it's own consent form, I know ours do

That sounds promising. I'll ask next time I'm there.

My husband did it verbally in my presence and it was put in my husband's GP notes, also his hospital notes.
