It might be piles.
But how the hell can a telephone conversation determine that?

Ive already had a colonoscopy where pre-cancerous bits were cut out and another where internal piles were injected.

Sounds like you definitely need to get a stool sample to the surgery before midday to be sent off for analysis and a follow up colonoscopy.

Could be a fissure then that opens up when you force yourself and bleeds. Get it checked out because if it gets really bad, it might require an operation.
