Stapedius, 5mm long, in the inner ear.
I think the stapedius is a bone in the inner ear, along with the incus and malleus. The eyes certainly have muscles surrounding them - perhaps one of them?
The smallest muscles (like the smallest bones) are in the inner ear. They're the stapedius and the tensor tympani which allow for clear hearing as it's a delicate apparatus.
