Find a doctor's surgery nearer to where you live and contact them to see if they are willing to take on new patients. If they aren't oversubscribed they should agree to take you on. If they aren't taking on new patients, they may have a waiting list, or you could try another surgery. If they can take you on they should request your medical file from your previous GP for you.
when i moved here in march, i was told there were rules in place with the local health authority and noone could be with a doctor outside of the set boundaries, and so you have to be accepted by a doctor in your boundary.
go to www.nhs.uk and type in your postcode on the rt hand side and click gp. it will come up with a list in nearest order. pick the one yu want and ring them up to see if you are in the boundary then just go and register
