I can't imagine even a risky inmate trying to do a runner or whatever while in labour. I would hope maybe they were in a secure, locked room with a guard on the door if they were a serious risk to anybody.

However, in Britain, women prisoners have NEVER been chained during childbirth, or at least never in the past few decades. The suggestion is offensive, meritless garbage.
I challenge anyone to cite any credible source which claims otherwise.
However, some women prisoners are 'chained' on hospital treatment visits; if a risk assessment suggests that they may try and escape.
