the burger chains train their staff to rigouorus standards, in the same way, in every restaurant throughout the world. they are trained to sell 'the product' in a specific way, and to offer customrs ' the product' regardless of what they ask for. So, they may sound a little robotic, and appear not to understand, but in reality, they are just doing as they are told - on pain of the sackif they don't!
It's what they're trained to ask and are then on automatic pilot. And they only hear part of your request, e.g. I clearly say "a quarter pounder meal to take out, please" and they ask "do you want to eat in or take out", having switched off halfway through my request. My wife once ordered a black coffee only to be asked if she wanted milk with it.
