mateojc- Looks like you've had a tough time getting an answer to this one! Well, you're in luck, I have recently stubbled onto this site (from the U.S.)..and my brother was a US Navy submariner for 4 years...I asked him and here is his answer to your question:

This I know very well, mainly because I could hardly do anything on my own wothout one.  The purpose is to respectly request something from your chain of command.  Each person appointed over you has to sign it before your request is approved.  They can also disapprove it.  I have written one for special liberty, swapping duty days, and taking leave of absence to see family and friends.  Basically it is to inform who you work for what you plan on doing, and if it conflicts with their plans for you to work or whatever the case may be it will not be allowed to happen.  People in the military have to write a request chit to get married...
