there isn't one, it depends on which organisation you work for. They all follow the same general lines but will vary in detail. can you say which organisation you work for?
Also the policies should be freely available for all staff to see and have copies of if they wish, should also be available in braille and spoken form for folk who need it.

It's probably mainly similar to the ones above but with some special 6 finger and toes clauses written into it then.
