Hi Amy, I don't know if it's a common practice in the US but over here, we never send a photograph with a c.v. for a job application - it doesn't matter to prospective employers what you look like, they look for your skills and experience. As the others have said, you need just head and shoulder - no chest - and preferably full face. It would be better if you didn't try to take the photo yuorself, IMO, as you're often at an angle when you do. A passport-type photo would be better for this purpose.

Also living in the U.S., I would find it strange to find a photo in any resume/application. Actually, it puts the job offeror in a sticky situation since among the questions that a prospective employer can't ask is the sex of the applicant (along with age) since that's seen as possibly discriminatory… political correctness run amuck, of course, but it is what it is...
