For more on marking an answer as the "Best Answer", please visit our FAQ.yes no problems here, just printed them off, and took my time filling them in, you just have to get all the forms signed by a doctor or some such person, my mates daughter is a dentist ,so she did it for us,be warned there are a lot of signatures required, so take you time and you should be ok, you will save more than half of the cost by doing it youself
I just received the forms from the O.P.G (free and postfree) everything is explained very clearly in the guide booklet which came with them. The way I read it is that you can get someone who has known the donor for more than two years and has met and talked with the donor frequently, to sign, doesn't have to be a doctor. In fact one of the examples it gives you is something like -Have known John Doe for X number of years and we meet in the pub once a fortnight. I was quite surprised at that, unless I am reading it wrong knockavacc? The O.P.G will send you the forms for Health and Welfare at the same time if you need those too. -rosy
