Sign Off My Assessment Report Without My Consent

I have a work place assessment done from a outsource assessor due to suffering RSI at work, the assessor made a report and requested me to sign off, but something in the report he wrote was not what had been done during the assessment, and it's an important issues. I spoke to my supervisor about these and requested to discuss the issues further before I sign off the report. But my supervisor not listening and sign off the report on behalf of me and HR closed the file. It seems not right to sign off the report without my consent esp it's health issues yeah? I can't talk to HR as they decided closed the file. Therefore, it would be much appreciated if anyone could give me some advice. Thank you in advance.

When I disagreed with a work assessment many years ago I simply refused to sign it. Put under pressure by my manager and his, I was told it was just to show I'd seen it: I didn't care, I wasn't signing something I disagreed with. They stopped badgering me after a bit.

You write a letter
get a friend to help - in time order - to HR
1. Your assessment was done wrong
2. you pointed it out
3. the file was closed anyway
4. you wish the dissent ( by you to be recorded).
wait - if they say it cant be recorded then
write another letter
saying you raise a grievance over the handling of the data/letter
and requiring your dissent be recorded.
I think this covers all bases
yes it is important to say you dont agree at the time
