If you go to the Forms Section, it's Form TR1.
There is also an office finder so you can find the right Land Registry to send it to.
The Land Registry now have identification requirements for people submitting transfers without legal representation so give them a ring and check what you need.
There is a practice guide which confirms what is needed so I will try and post a link when the site is back on line.
Depending on your age and situation, make sure that you have received the proper advice about transferring the property away with regard to funding for any future care you made need and the ramifications if you should be made bankrupt within 6 years after the transfer.
