Ava wasn't in fact having an incestuous affair with her son, as he wasn't her son at all. She couldn't have children because she used to be a man. (Whilst a man, he had fallen in love with an ace plastic surgeon (Alec Baldwin) who turned him into a woman as he - the surgeon - wasn't interested in guys, but felt that he might be able to love Ava as a woman).
