Missed the start can anyone tell me why Billy had to have that operation? they mentioned an accident with a bike and then they were talking about a tumour so I was confused.

Billy did have a tumour and was due to have the operation, yet when he thought about what would happen to him he went out on his bike and deliberatly crashed, hoping that he would kill himself. And then both he and his wife wouldn't have to go through the trauma of the operation and then have to deal with how he was left physically.
