We know about the cancer dead/ not dead man/ killer - 1. Why does he want to do that to the two men? 2. Does the Dr who is asked to kell the 2nd man actually kill anyone? 3. Does anyone get 'sawn'? 4. Is the psycho cancer man still alive at the end and do the other two men know that it's him? Thanks!

The cancer man been doin all killings and he does them in way that he thinks helping people get lives back on track e.g. the girl who survived was druggie+ cancer man thought he was teaching her lessson 2 appreciate life.By surviving + doing horrible thing she had 2 do was prooving she was strong enough.No the doc does not kill anyone.He shoots Adam but as we know he didn't die.The person to get sawn is the doc who has 2 saw foot off. He has 2 do this as last attempt 2 save his wife + daughter, but we know they are already safe. He has 2 kill Adam 2 save his family. Irony lies in that cancer man at end tells Adam key was in bath + if hadn't pulled the plug none of this would have happened as they could have escaped, and got gun from apparent dead man - unless he got it + killed them.The cancer patient is still alive at the end as we c him walk out the room and he says "Game Over" to Adam. Adam is left in the room presumably to die unless doctor can escape in time and doesn't bleed to death, so he can get help or Dr might get caught by the cancer man who probably would be faster than the doctor who was in a pretty bad way, mentally and physically.Adam is the only person left in the room along with hospital worker who he has killed with the top of the toilet cistern (or so we assume) Adam won't know who the cancer patient is I don't think as probably hasn't been in contact with him before. If I remember rightly he was asked to take photos of the Dr who was having an affair but I am unsure if he was asked to do this by the cancer patient man or his sidekick the hospital worker. Obviously the doctor would know the cancer patient as he treated him in hospital which u c in earlier scene. Dr doesn't recognise him on the floor as has a bloody mask on.

The only bit I don't understand is how the hospital worker got involved. In the hospital scene you see him saying to the doc "he's a very interesting person" so I gather that they have got talking in hospital and have planned all the crimes or the worker has got involved in the killings as he believes in idea that they can teach people about the meaning of life. How does the cancer man get into the room tho? One minute he is dying in hospital and the next he is playing games lying in the middle of the room pretending 2 b dead.

Sorry for extra long answer. Tried to cut it down but didn't really work. This is how I interpretted it - might be wrong!

1) He wants to do that to the Dr because he is having an affair with someone behind his wife's back and when the killer was in hospital and the Dr had the students in he kept referring to him as "the patient" (rather cheeky!) and not using his name even when prompted to by the bloke who ended up being blackmailed into keeping the wife and daughter hostage by the cancer man (real killer) - still following??????  I reckon the killer is a bit a bit like the killer in 'Se7en'.  He sees things that are wrong in people and tries to 'straighten them out' in his own sick way.  I think the other man (the photographer) was targeted because the job he does is seedy and prys into people's lives.  He snoops around and spies on people and gets paid for it (ironic really that the killer is punishing him for prying into the life of the Dr who he is trying to hurt as well.).  It's also a strange kind of thing because the killer hears the Dr speaking about him disrespectfully as a patient, the hospital orderly guy (who is later blackmailed) defends the patient/killer, the killer is peed off with the Dr so blackmails the orderly guy who plants his torch/pen at a murder scene. This prompts the detective to investigate the Dr who pays the photographer to follow the Dr.  If you think about it, the Killer has created the whole sequence...

2) The Dr kills no one.  He merely shoots the photographer bloke in the shoulder so it LOOKS like he has shot him to the killer 

3)  The Dr saws his own leg off so that he can take the bullet from the box and put it in the gun that the 'dead' killer guy has in his hand and shoot the other guy (as above)

4)the guy was alive all along  The Dr probably gets killed by him because he wouldn't have been able to escape up ladders with no foot (sawn off). The other guy is killed

Danny Glovers character pays Adam to take the photos of the Doc.One question i have is would the police not be able to trace the signal from the phone?.Remember the Doc speaks to his wife & daughter while he is trapped in this room.If so then would Adam not get saved?.
