There is something very sinister about this story (if it is indeed true). What kind of person would do something like this - a narcissist?

it's weird, sinister and totally unprofessional. I wonder what other "silly" things he's done that haven't been discovered.

Sara - these are called sentinel events -
and gives the employer to trawl back through all the notes
looking for others
or to interview the other staff and invite them to "spill the beans"

The idea is that sentinel events lead on to Big Events

and unfortunately (for the amount of money spent on this rather than caring for you and I) sentinel events may be random....
in which case the look-back exercise is a complete waster of everyone's time and money. - as all you identify is a non-cluster of random events.
revisiting this story as I just wanted to add The surgeon concerned is a brilliant man and a lovely person I have been his patient and worked in the same unit... I would still want him to be the man with the laser on anyone I loved and would trust him completely. I am sure he is by no means the first to do this just the first one there has been a story about. I just hope he has his knuckles rapped and he is allowed to get back to performing life saving surgery.
