Studies have shown that people whom are considered poor quite often have a diet and or exercise regime that puts them at a higher risk of dying from cancer, heart disease, etc.
Maybe due to a lack of education or maybe their income can't cover the costs involved in creating nutritious meals, visiting the gym or treating the problem.

I think money figures because lots of poorer families are too stressed and otherwise overwhelmed to consider good nutrition, they want something cheap and easy, plus cigarettes are one of those horrible props like alcohol and drugs that seem to soothe some people's horror at their own condition, so again more likely amongst poorer people I would think who are perhaps a little less educated health care wise about long term problems it might cause.

I would have thought that that`s pretty obvious. Richer people are more likely to have private health care (in the US and here) and things get picked up sooner. Why do you think the Royals live so long? They wouldn`t live so long if it took four visits to the GP to peruade them that they need to take their symptoms seriously.
