It is indeed Czech, but my Czech is a bit rusty so I'd need a bit of time to sit & work it out. There are some hačeks (the little "v" over some letters, ilike the one in "haček"!) missing, but that shouldn't be an insurmountable obstacle. I'll get back to you - unless a fluent Czech-speaker beats me to it!
