I'm all filled in and have the cryptic clue (from the extra letters) have the fifth thematic item. But, now I'm stuck with what to write below the grid.
All I can come up with is the last word. Could it be MOD are the first letters of the expression?

Last niggle is 34 down, I'm pretty sure I've got it right but don't understand the wordplay.
Actor John forgetting bill: sentational rush (5) _UR_Y (the extra letter is T) and I hope that it's 'hurry'

Sensational is RACY. Forget 'bill' AC, so you get RY.

Add together and you get HURRY.

I know it was a bit feeble but it was late and that last glass of Jameson made me do it. I also thought maybe some other people might have liked to enter into the spirit - seemed a bit sad to let him pass without a bit of a suitable epitaph.
