It does actually only take a couple of minutes.
Your question lacks the option to choose to live. It assumes you are compelled to take a risk that in real life most people would avoid if at all possible.

Maybe you should include something that says you are compelled to participate and only have the two choices offered. Which would you choose?

Guys, it's a general consent form for the lab that I'm doing my thesis in. It doesn't taken an hour. Only 2 minutes. I promise.

Yes rojash, I had no concerns about confidentiality as no personal info was requested. I think the 'less than 1 hour' statement, though correct, is not a good one- less than 5 minutes would be far better
