Name Change Confusion!
8 Answers
After being divorced several years, I've decided to revert back to my maiden name. How should I go about this?I've read that I need a copy of my marriage and decree absolu certificates to send out as proof, but I have lost both of these. Should I just pay for a deed poll to save all the hassle? Cheers

You do not need anything other than the appropriate form. If you decide to do it by Statutory Declaration rather than Deed Poll then buy the form from someone like this firm, fill it in yourself, then pop into any solicitor who will charge you 5 to witness it and make it legal.
