Tilly it's now 4-1 to Inter so 4-4 aggregate score but Spurs have the only away goal in the tie. There's just under 10 minutes left.

Yes - if the aggregate score is tied at the end of the 2nd leg then the team with more away goals go through. But in this game the score after 90 minutes was 3-0 (3-3 aggregate) so no away goals had been scored at that point. That's why extra time is now being played and Spurs goal during it ensures that penalties will not be needed.
