Is anyone else having problems getting their Monarch ATOL claim form from the CAA (or the company they have contracted out to do this for them)? I've called 5 times now and they keep saying they'll escalate it but nothing forthcoming. Promised on 11th, 16th, 20th, 23rd and 26th October.

i would imagine they are innundated. As it's only been 2 weeks, you are perhaps being optimistic. In answer to your op, as mine was only flights, i am claiming from my CC, who have already "temporarily" credited my account with the amount and monarch has 60 days to dispute it
Have you checked your junk email folder?

Thanks. Always optimisitic!! Just wish they'd say instead of promising and not delivering (although it was 2nd October which is closer to 4 weeks now). I've checked junk, deleted, every folder in my inbox. Received immediate refunds from various other companies following our cancelled holiday but the I guess the CAA probably don't have the same resources as the private sector.
